Who Is Eligible?
- Individuals whose personal information was compromised in the February 2023 Proliance Surgeons data breach
- Those who received a notice from the settlement administrator with a Class Member ID
- If you didn’t receive a notice but believe you were affected, you can still file a claim by mail
Settlement Payment Options
- Out-of-Pocket Expense Claim (up to $5,000): Reimbursement for documented expenses caused by the breach, including bank fraud fees, credit monitoring costs, ID replacement, legal/accounting fees, travel expenses, and more — with supporting documentation
- Pro-Rata Cash Payment (~$559): Available to all eligible class members regardless of documented losses — no proof required. The exact amount will depend on the number of valid claims filed.
